Instructions for Submitting Email Authorization Form
- Please download the email authorization form.
- Fill out the form and make sure you sign it.
- Scan the form so you can upload it. When you scan the file save it as either a PDF or a JPEG image. If you do not have a scanner then you can use your smart phone to scan your document.
